Back @ Work™ is an out patient assessment and treatment program provided by a multi-modal team that includes pain physicians, physical therapists, occupational therapists, psychologists, and pharmacists for workers with complex pain issues
All workers referred by their family physicians, undergo an Interdisciplinary Pain and Medication Management Assessment by trained pain physicians to provide treatment recommendations. Treatment services may include:
- Pain and Medication Management Assessment
- Complex Pharmaco – Therapy management
- Nerve blocks, trigger point injections, epidurals
- Intravenous chronic pain medications
- Pain Management Modules (a group education and activation program)
- Customized services (individual session with team members as needed)
- Return to Work planning and monitoring
- Post-discharge follow-up
