The Pain and Medication Management Assessment is a five part customized review designed for each patient’s specific need.
A Physician Assessment comprised of:

A detailed Medical History;

Physical Examination;
-
An assessment of all current medication taken by the patient;

Communication with the family physician to build consensus on the treatment plan.
A Physical Therapist Assessment
- Identification of subjective and objective musculoskeletal and neurological findings related to the diagnosed condition;Identification of any physical or associated factors which may impact return to work;
- Confirmation of the patient’s general fitness for participation in a treatment program;
- An assessment of the patient to develop the appropriate physiotherapy treatment plan.
Occupational Therapist Assessment

Functional assessment to confirm the patient’s current level of functional ability and activities of daily living through standardized testing or self report;

Functional assessment relative to critical job demands of the pre-injury job may be appropriate for patient’s who are job attached and functional testing is determined to be meaningful;

For non-job attached patients, functional testing shall focus on the functional tasks as related to the patient’s previous critical job demands, future vocational direction or daily activities as appropriate;

Functional testing shall be tailored to prevent excessive symptom aggravation
Psychological Assessment
- A brief history of the presenting complaint;A review of relevant past psychological history and current social and vocational functioning (including review of any pertinent records and documents);
- A structured interview and/or investigation to assess any relevant emotional and personality conditions;
- An evaluation and interpretation and scores of at least one (1) standardized psychometric instrument (Examples are Pain and Disability Index, Beck Depression Inventory or Strait-Trait Anxiety Inventory);
- A statement about prognosis;
- Comments on the validity of the clinical presentation indicating if symptoms presented were plausible, straightforward, under-reported or over-reported;
- Treatment and rehabilitation recommendations;
- DSM IV diagnosis or similar integrating opinion;
- Feedback to the patient on the assessment process.
Medication Management Assessment completed by a Pharmacist if an opioid or sedative-hypnotic medication issue has been identified in the referral and is comprised of:
- A history of the past and present medication regime;
- Recommendations on medication management.
